Prior to the start of the Fall season your LCHA and USA Hockey registrations must be complete and submitted to the LCHA The State of Michigan requires you to view the Heads Up Concussion Training Course and provide a copy of your completion certificate. In addition, the LCHA Registration documents below are required by each player before the player can be rostered as part of the team. You should receive hard copies of these forms from your team's coach or manager prior to the start of the season and they are to be returned to the manager for submission. Players new to the LCHA will also be required to submit a copy of their birth certificate with their registration.
Except for cross-ice and mite teams, no one is permitted in the locker room unless they have completed a MAHA background check, Safesport training and are registered with USA Hockey. It is the policy of USA Hockey, MAHA and the LCHA that there are at least two responsible adults directly monitoring the locker room during all team events to ensure that only participants (coaches and players), approved team personnel and screened family members are permitted in the locker room and to supervise the conduct in the locker room.
Cell phones are prohibited in the locker rooms – no exceptions.
